It's a aftermarket, not stock, EXHAUST MANIFOLD for a Mitsubishi 4G63 TURBO engine. The extra outlet (round, two bolts) is for a AFTERMARKET exhaust pressure relief controller. The standard relief valve is below the manifold, incoporated into the turbo downpipe.
