This is a FLYWHEEL TURNING TOOL. It's used to turn auto and truck flywheels when removing/installing clutches. Flywheels have teeth around the outside for the starter to engage. This tool hooks on those teeth to turn (or hold it from turning). Clutch pressure plates have bolts around the edge that are removed, and then tightened, when installing a new clutch.
