What you have appears to be a wall mount sconce for lamps. I posted a link with a picture below. Its a pulley designed for lowering & raising a light bulb with a rope/string

It is a vintage guide wheel for an elevator, it used to attached to the side of the passenger car and Ride Along the side of the shift to keep it from twisting as the cable would pull the car up and down.
